Procedure To Obtain A Health License:

All items must be submitted in writing:

Submit a copy of Health plans -To be reviewed by the county Health Inspector -The Health Dept. Must have there own copy of health plans.

Submit a copy of resolution
Submit a copy of CO
If you cannot supply this please explain

Submit copy of your food establishment menu

Name and address of the food establisment you will be buying
Name and all addresses of people who are owning the business
Name and addresses of establishments you will be purchasing your food from for your business

Must pay Health fee -Will pay with a check or money order the night of the Health Meeting -No Cash!!
Fill out Health Application **You will receive this the night of the meeting.

All owners must attend the health meeting** If anyone does not speak or understand english they must come with someone that does.

No food establishment can open in the Boro until all of the Health requirements are completed and sent to the Health Secretary.
If for any reason you cannot supply any of the above items please indicate in writing why.

A Health Inspection will be given by the County Health Inspector before a Health license can be issued. The food establishment must pass the Health inspection in order to obtain a Health license. All employees for the food establishment must have completed the proper food classes, food managers, and food handlers. The food establishment must also obtain all the other permits from the other departments in the Boro.

If all of the requirements are completed before the regular schedule Board of Health Meeting the secretary will conduct a telephone poll to the board members on granting the food establishment a temp Health license until the Board meets.
